Job Summary This pharmacist will provide pharmacy services primarily at Upstate’s Cancer Center Verona (as needed at our other regional Upstate oncology clinics located in Central New York). The Upstate Cancer Center and affiliated clinics provide state‑of‑the‑art cancer care to the patients it serves. Under the direction of the hospital’s Pharmacy Department, the Cancer Center Pharmacy functions via a dedicated pharmacy space with its own modern sterile products laboratory. The applicant will work with a team of oncology‑trained pharmacists and technicians to manage the preparation and oversight of oncology treatments, including patient profile review, sterile and non‑sterile medication validation/preparation, patient counseling, formulary/inventory management, and staff/provider communication. Job duties for this position also include but are not limited to:
Interpretation of medication orders and prescriptions to ensure that they are complete, safe, rational, and compatible with institutional policy. Preparation of compounds and dispensing medications for inpatient and ambulatory care use, including injectable sterile products. Dispensing pharmacy‑related items. Performing duties in all areas of Pharmacy, including inpatient, ambulatory care and satellites, primarily in the ambulatory care clinic. Performing duties in various practice locations, including inpatient and transitions of care settings based on department needs. Providing information to health‑care practitioners to promote the rational use of medications. Providing drug‑monitoring services. Maintaining pharmacy records by law and institutional policy. Interpreting policy and procedures for hospital staff. Participating in teaching programs for hospital staff. Participating in Pharmacy Department’s Quality Assurance Programs.
Minimum Qualifications
B.S. in Pharmacy or Pharm.D and NYS Pharmacist license and registration (or eligible) required. Candidates who possess a NYS pharmacist limited permit must obtain license within 1 year of appointment.
Preferred Qualifications
Previous hospital experience preferred. Previous pharmacy oncology experience or completion of a PGY‑2 Oncology Residency preferred. Previous pharmacy sterile products experience preferred.
