Position Overview:
The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching. This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities.
Key Responsibilities:
* Provide direct patient care under the supervision of RNs or physicians.
* Conduct health coaching sessions to promote wellness and chronic condition management.
* Perform accurate medical documentation in compliance with healthcare regulations.
* Assist in outpatient settings, LTC, and hospital medicine environments.
* Administer medications and treatments as prescribed.
* Monitor patients’ vital signs and report any changes.
* Support patients undergoing dialysis and educate them about the process.
* Utilize medical terminology to communicate effectively with healthcare teams.
* Conduct diagnostic evaluations to assess patients’ health status.
* Ensure infection control protocols and maintain a safe patient environment.
Requirements:
* Active LPN License in the state of practice (VT, NH, or compact license).
* Minimum 2 years of Long-Term Care (LTC) experience.
* Must be able to adapt quickly and work with minimal orientation.
* Experience working in memory care settings preferred.
* Excellent communication skills – verbal and written.
* Ability to work collaboratively while maintaining patient confidentiality.
* Flexibility to adapt to varied healthcare environments and schedules.
