Ph3Position Overview /h3pThe Licensed Practical Nurse (LPN) is an essential part of the team responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians' orders for each individual patient while adhering to confidentiality standards and professional boundaries circulate at all times. /ph3Territory /h3pDane County, West side - Middleton, Verona, Belleville, Fitchburg, West Madison, East side - Monona, East Madison /ph3Schedule /h3pMonday through Friday and 7 day on call rotation /ph3Essential Job Functions /h3ulliDeliver quality patient care in compliance with physician orders under direction of their Registered Nurse teammate. /liliPerform the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers. /liliRecognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk. /liliEducate clients and their family members based on client's specific needs. /liliProvide care in patients' home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc. /li /ulh3Requirements /h3ulliAn active LPN License in the state of application. /liliValid CPR. /liliValid Drivers License. /li /ulh3Preferred /h3ulliMedicare Skilled Nursing experience. /liliBasic understanding of Oasis. /lili1-year LPN experience in a home health or healthcare setting. /liliHome Care Home Base experience a plus. /li /ulpAs an employer accepting Medicare and Medicaid funds, employees must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate. /p /p #J-18808-Ljbffr
