Job Summary:
The Physical Therapist shall screen, evaluate, re‑evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
Responsibilities/Requirements:
* Screen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy intervention is indicated.
* Evaluate residents per MD order and establish appropriate Treatment Plans and Goals that meet the specific needs of the individual.
* Reassess patients and caseload at least every 30 days to determine the need for continued services or discontinuation of services based on patient progress. Carry out treatment plan as indicated.
* Supervise treatment implementation carried out by Licensed Physical Therapy Assistants and determine appropriateness, as well as modify treatment plan as indicated.
* Provide thorough discharge planning that will allow the patient to function at a maximal level of independence in the least restrictive environment.
* Educate staff, family, and care givers as indicated.
* Identify needs for program development and implement programs according to company policies and procedures.
* Identify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy.
* Complete all required documentation according to guidelines specified in Documentation Manual.
* Coordinate in‑services for facility staff as need is determined, to ensure safe and effective carry over of Maintenance/Restorative Programs and overall call of the residents.
* Attend all facility clinical required meetings (Medicare, Care Plans, etc.).
* Promote profession within facility, including assisting with coordinating special events i.e. National Physical Therapy Month.
* Attend Continuing Education courses specific to current position.
* Maintain professionalism and always follow all national/state associations regulations.
* Supervise student affiliates as directed.
* Assist employees in the absence of Clinical Manager.
Qualifications:
* Bachelor's, Master’s, or Doctorate in physical therapy from an accredited program.
* Certification and licensure requirements as established by state and national board associations.
* Active state license required.
* Excellent organizational and time management skills.
* Excellent written and verbal communication skills.
* Possess current working knowledge of Functional Pathways policies and procedures, and clinical programs with application skills to therapy disciplines.
* Ability to supervise Physical Therapy Assistants.
* Completion of all required screenings, certifications, and licensure. Screening requirements may vary by location.
