Registered Practical Nurse/Case Manager, HIV Program

1 FTE Permanent

WHIWH CHC provides racialized women, trans and non-binary clients from the African, Black, Caribbean, Latin American, and South Asian communities in Toronto and surrounding municipalities with culturally safe, relevant, and responsive primary healthcare.
We are changemakers engaged in knowledge transfer and exchange, research, capacity building, and advocacy within and across multiple systems.

Position Description

Women's Health in Women's Hands CHC is recruiting a highly motivated, professional, and dynamic individual to join the team:

The position includes coordination and delivery of services for women living with HIV, ensuring high-quality and personalized care, managing client relationships, assessing needs, developing care plans, and ensuring compliance with organizational policies and standards.
The incumbent will serve as a liaison between clients, staff, and external providers, addressing concerns, monitoring outcomes, and facilitating continuous improvement in care delivery

Accountabilities - Key

Conduct intake, needs and initial medical assessments for new clientsCoordinate clinical, allied health and community-based care/supportCoordinate appointments for interdisciplinary care for women already in care and those entering careRespond to all requests for care coming from multiple sources including self-referrals from women living with HIV and referrals coming from hospitals, Public Health nurses, immigration doctors, family doctors, AIDS service organizations and other community-based settingsFollow up on all referrals inside and outside and close the referral loop to support optimization of health outcomes for women living with HIVLiaise with WHIWH HIV testing programs to support entry into care for women diagnosed through these programs.Develop and support implementation of interventions to reduce no shows and loss to careSupport the HIV care team in coordinating services and care for clients with complex needsSupport monitoring of care plans and development of strategies to optimize care for all women living with HIV.Coordinate access to treatment for women without health coverage through pharmaceutical compassionate drug access programs and other means where available.ensuring care requirements are met, and that client care issues and concerns identified by women living with HIV and their families are addressedCreate and maintain a supportive environment for women living with HIV and staffSupport development and monitoring clinical performance metrixs and implement strategies where they fall shortCollaborate with a multidisciplinary team of care providers to achieve operational and Client-focused care objectives within the HIV Program.Provide leadership for the entire HIV care team and co-chair the monthly HIV Interdisciplinary team meetingsSupport peer workers and team developmentManage program budgets and resourcesCoordinate quality improvement plans for the HIV programGenerate progress reports for management team as requiredCoordinate writing of funders reportsSupport program growth and expansion plans.Various nursing responsibilities within scope of practice; vaccinations, health assessments, etc.Any other duties as deemed necessary.

Qualifications

Demonstrated experience working as an RPN in a community and/or public health setting.Registration as a Registered Practical Nurse with the College of Nurses of Ontario.In-depth knowledge and understanding of racialized communities, particularly the African and Caribbean communities in Toronto;Thorough knowledge and understanding of medical and laboratory terminology, medical record procedures and charting, universal precautions, and other current health and safety hazards.Knowledge of anti-black racism and its impact, particularly on the African, Caribbean and Black communities.
particularly womxn;Experience in service provision for racialized communities;In-depth understanding of services available for women living with HIVAwareness of anti-oppression issues (e.g.
sexism, anti-choice, homophobia, trans-phobia, racism);Strong organizational and communication skills;Ability to work well with diverse groups;Computer literacy skills;Experience in report writing;Knowledge of womxn's health and disability issues is an asset;

Reporting

This position reports to the Manager, HIV Prevention and Care Cascade Program
