Particularly in skills review, in utilizing the electronic medical record appropriately and in the adherence to agency policies and procedures. Provides direct care as part of maintaining individual skills, and for the purpose of mentoring others. /ppustrongPerformance Responsibilities and Standards: /strong /u /pp1.Ensure quality and safe delivery of home health services. /pulliInstruct new staff in agency policy and procedure /liliVerify appropriate technique in demonstration of new skills in both the home and simulated setting /liliProvide support and ongoing education for existing staff with identified performance deficits /liliProvides direct patient care as appropriate to meet agency needs or as demonstration to staff. /li /ulp2. Provide individualized guidance to clinical staff. /pulliThru joint visits, direct observation, chart review, and other methods as needed to track development of new staff or existing staff /liliEnsure compliance of new staff or existing staff under with established standards of documentation to provide evidence of skilled, medically necessary intervention. /liliEmploy the principles of adult learning to convey new or changed information or to elicit performance improvement with staff /liliInstruct staff in principles of time and utilization management to deliver efficient, effective care /liliMentor new staff in the role and responsibilities of a case manager /liliProvides input to aid in the completion of the new hire evaluations for clinical staff at 90 days and advises to extend probations if necessary /li /ulp3. Participate in personal growth and development. /pulliParticipate in educational in-services and meetings. /liliMaintain and update clinical knowledge on an ongoing basis. /liliPerforms all other duties as assigned /li /ulpustrongRequirements: /strong /u /pulliCurrent New York State Registered Nurse professional license. /liliDegree from an accredited school of nursing or college. /liliOne-year experience as a public health nurse in a home care agency. /liliCurrent and valid driver’s license and auto insurance and a vehicle which can be used for work. /liliSelf-directed and organized with the ability to work independently. /liliGood oral and written communication skills. /liliBachelor’s degree or higher in nursing or related field preferred /li /ulpustrongPhysical Requirements: /strong /u /pp1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication. /pp2. Ability to perform tasks involving physical activity including: /ppLifting and carrying- typical weight 25 pounds; maximum weight 40 pounds /ppPatient transfers, extensive bending, standing, kneeling, walking in various weather conditions, e.g. snow, rain, sleet and going up and down stairs. /pp3.
