Job Summary: We are seeking a highly motivated and experienced Registered Practical Nurse to join our team as a Primary Care Nurse Specialist. The successful candidate will be responsible for coordinating and delivering high-quality care services to women living with HIV, ensuring personalized attention and addressing their unique needs.

About the Role:
">
* The Primary Care Nurse Specialist will conduct initial assessments, develop care plans, and ensure compliance with organizational policies and standards.
">
* They will serve as a liaison between clients, staff, and external providers, addressing concerns and facilitating continuous improvement in care delivery.
">
* The role involves coordination of clinical, allied health, and community-based care/support, as well as responding to referrals from various sources.
">
* The nurse will also support the development and implementation of interventions to reduce no-shows and loss to care.
">
* They will work collaboratively with a multidisciplinary team to achieve operational and client-focused care objectives within the HIV program.
">
* In addition, the Primary Care Nurse Specialist will provide leadership for the entire HIV care team, co-chairing monthly interdisciplinary team meetings and supporting peer workers and team development.
">

Key Responsibilities:
">
* Conduct intake, needs, and initial medical assessments for new clients.
">
* Coordinate clinical, allied health, and community-based care/support.
">
* Liaise with WHIWH HIV testing programs to support entry into care for women diagnosed through these programs.
">
* Develop and implement strategies to optimize care for all women living with HIV.
">
* Ensure care requirements are met, and that client care issues and concerns identified by women living with HIV and their families are addressed.
">
* Create and maintain a supportive environment for women living with HIV and staff.
">
* Support monitoring of care plans and development of strategies to optimize care for all women living with HIV.
">
* Coordinate access to treatment for women without health coverage through pharmaceutical compassionate drug access programs and other means where available.
">
* Manage program budgets and resources.
">
* Coordinate quality improvement plans for the HIV program.
">
* Generate progress reports for management as required.
">
* Support program growth and expansion plans.
">
* Various nursing responsibilities within scope of practice; vaccinations, health assessments, etc.
">

Requirements:
">
* Demonstrated experience working as an RPN in a community and/or public health setting.
">
* Registration as a Registered Practical Nurse with the College of Nurses of Ontario.
">
* In-depth knowledge and understanding of racialized communities, particularly the African and Caribbean communities in Toronto.
">
* Thorough knowledge and understanding of medical and laboratory terminology, medical record procedures, and charting, universal precautions, and other current health and safety hazards.
">
* Knowledge of anti-black racism and its impact on the African, Caribbean, and Black communities, particularly womxn.
">
* Experience in service provision for racialized communities.
">
* In-depth understanding of services available for women living with HIV.
">
* Awareness of anti-oppression issues (e.g., sexism, anti-choice, homophobia, trans-phobia, racism).
">
* Strong organizational and communication skills.
">
* Ability to work well with diverse groups.
">
* Computer literacy skills.
">
* Experience in report writing.
">
* Knowledge of womxn's health and disability issues is an asset.
