The West Toronto Diabetes Education Program (WTDEP) educates and supports individuals at risk or living with prediabetes or type 2 diabetes over the age of 18. Our program spans 5 community health centers in West Toronto and Mississauga regions. Job Summary : In this role, you'll support and educate individuals at risk or living with prediabetes or type 2 diabetes, helping them manage their condition and improve their quality of life. Collaborate with healthcare professionals to create care plans, lead educational sessions, provide individual counselling, outreach, and promote healthy lifestyles. The incumbent will ensure adherence to organizational policies and maintain standards aligned with the program and College guidelines. Primary Responsibilities : Provide care within your professional scope for clients at risk or living with prediabetes or type 2 diabetes, considering their knowledge, readiness to change, lifestyle, and socio-demographic background. Develop health maintenance care plans with clients and the healthcare team, integrating health status, clinical guidelines, and community resources. Identify and prioritize complex health and social issues. Ensure accurate, complete, and timely documentation of activities and medical records. Participate in chart reviews to monitor quality and thoroughness. Communicate clearly and effectively with clients and healthcare providers. Experience with case management and coordination within an interdisciplinary team. Create and facilitate educational classes and practical sessions on topics related to diabetes, such as cooking workshops or physical activity classes. Build relationships with community partners. Present updates on clinical practice guidelines to healthcare providers and partners. Provide learning opportunities for students, new hires, and colleagues. Recommend procedural or service improvements. Promote teamwork and group effectiveness. Respect diverse backgrounds and perspectives. Participate in committee work to improve programs and client care. Perform other duties as assigned by the manager. Required Qualifications : Bachelor’s degree in Human Nutrition, Kinesiology & Health Science, or related health field. Must be a Regulated Health Professional in Ontario (e.g., Registered Dietitian, Registered Kinesiologist). Current membership in good standing with the respective professional college. Professional liability insurance as required by the college. Experience working in a community setting. Three to five years of experience, preferably in community-based diabetes care.
