We are seeking Registered Nurse to join our team in our Physician Support Nurse role servicing our patients throughout the PeeDee area. Physician Support Nurse role will provide direct clinical and care coordination support to physicians within the care group. The PSN ensures that physicians are equipped with accurate patient information, coordinated follow-up plans, and streamlined communication with the interdisciplinary care team. This role combines clinical expertise, patient engagement, and strong organizational skills to improve care quality, support population health goals, and enhance physician effectiveness in delivering value-based care.
This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

What Are We Looking For?
Your Health is currently looking for a Licensed Nurse to join our growing primary care family.
The following is a list of essential functions, which may be subject to change at any time and without notice. Management may assign new duties, reassign existing duties, and/or eliminate function(s).
* Serve as the dedicated nurse supporting the physician on the care group.
* Prepare and reconcile patient charts, ensuring all test results, care gaps, consents, and documentation are accurate and available prior to visits.
* Anticipate physician needs during patient care and provide clinical assistance as appropriate.
* Support diagnosis accuracy, documentation standards, and quality measure completion.
* Facilitate effective communication across the care group to align on patient care plans.
* Perform intake, vital signs, and medication reconciliation; document patient histories in the EMR.
* Provide patient education on care plans, conditions, medications, and self-management strategies.
* Monitor ADT alerts (admissions, discharges, transfers) for assigned patients; ensure timely physician follow-up.
* Coordinate with specialists, hospitals, and post-acute facilities to support smooth transitions of care.
* Schedule follow-up visits and preventive services to close care gaps and maintain care continuity.
* Support quality performance by tracking and closing care gaps, improving documentation, and promoting preventive services
QUALIFICATIONS
* Must be a licensed nurse. License must be in good standing with the appropriate board.
* A minimum of three (3) years clinical experience preferred.
* Demonstrated ability to supervise and direct professional and administrative personnel.
* Ability to read and communicate effectively.
* Strong written and verbal skills.
* Basic computer knowledge.
* Ability to manage and demonstrate effective leadership skills.
* Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
