Join us in making a positive impact on our residents' lives! /p h3Job Summary: /h3 pThe Physical Therapist Assistant shall implement the plan of care as established by the RPT and the physician in a professional, competent manner, upholding excellence with quality of care. /p h3Responsibilities/Requirements: /h3 ul liScreen all residents in facility on a quarterly basis, to identify those who have experienced declines in their functional abilities and determine if Physical Therapy intervention is indicated. /li liCarry out treatment program according to plan of care in consultation with Registered Physical Therapist. /li liIdentify the need for modification of treatment plan and consult with Registered Physical Therapist. /li liMonitor and report resident performance and progress. /li liAssist Registered Physical Therapist in re‑assessment of patients. Assist in determining need for continued services or discontinuation of services based on patient progress. /li liAssist Registered Physical Therapist in discharge planning. /li liSupervise Rehab Therapy Aides. /li liEducate staff, family and caregivers as indicated. /li liIdentify needs for program development and implement programs according to company policies and procedures. /li liIdentify equipment needs of Physical Therapy Dept. and order equipment according to company and facility policy. /li liComplete all required documentation according to guidelines specified in Documentation Manual. /li liCoordinate in‑services for facility staff as needed and determined, to ensure safe and effective carryover of Maintenance/Restorative Programs and overall call of the residents. /li liAttend all facility required meetings. /li liPromote profession within facility, including coordinating special events during National Physical Therapy Month. /li liAttend Continuing Education courses specific to current position. /li liAlways carry out all job responsibilities in a professional manner. /li /ul h3Qualifications: /h3 ul liAssociate degree in Physical Therapy Assistant program from an accredited institution. /li liCertification and licensure requirements as established by state and national board associations. /li liActive state license required. /li liExcellent organizational and time management skills. /li liExcellent written and verbal communication skills. /li liPossess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines. /li liCompletion of all required screenings, certifications, and licensure.
