The position involves home visits, counseling, service coordination, crisis intervention, and advocacy to help consumers access medical, social, educational, vocational, and financial resources. Most duties are performed independently with general supervision. /p h3Primary Responsibilities /h3 ulliConduct home visits to provide supportive counseling and independent living assistance. /liliHelp consumers modify behaviors, build insight, and develop coping skills. /li liAssess needs, gather medical/psychosocial information, and develop individualized service plans. /li liMaintain accurate case records, documentation, assessments, and quarterly reviews. /li liProvide 1:1 and group counseling using cognitive‑behavioral and skills‑training techniques. /li liCoordinate services with care managers, ICL staff, and external providers. /li liAssist consumers in accessing community resources, benefits, and vocational opportunities. /li liPerform crisis assessment and intervention; refer to appropriate services. /li liAccompany consumers to medical, social service, and government appointments. /li liReport incidents and complete incident documentation per policy. /li liConduct safety and cleanliness inspections of consumer residences. /li liPlan and facilitate recreational or social activities. /li liParticipate in meetings, supervision, and required training. /li liAdvocate for consumer needs within ICL and with external providers. /li liPerform other related duties as assigned. /li /ul pEqual Opportunity Employer /p pThis employer is required to notify all applicants of their rights pursuant to federal employment laws.
