Ph3Team Lead (RN/RPN) – Temporary 8‑Week Contract /h3 pLocation: 1767 Dundas St E, Mississauga, ON L4X 1L5, Canada /p pJob Type: Full Time (temporary 8‑week contract) /p pPay Rate: $35.80 – $41.96 per hour /p pApplication Deadline: 08/31/2026 /p h3General Responsibilities /h3 pThe HHP Team Lead supports the efficient and effective operations of HHP services and staff, supervising the Homeless Health Program, both in‑shelter and outreach interprofessional teams. /p h3Specific Responsibilities /h3 ul liReporting to the Manager – Clinical Community Care, supporting efficient and effective HHP operations. /li liSupervising the Homeless Health Program work, following planning guidelines and operational policies. /li liActing as a professional role model, mentor, and change agent for staff, demonstrating clinical expertise and ongoing professional development. /li liWorking with team members and management to meet staffing requirements for in‑shelter and outreach models, creating draft schedules for approval in alignment with employment standards and WellFort policies. /li liHolding regular team meetings and huddles to update team members on Key Performance Indicators and essential service delivery information. /li liProviding clinical leadership and direction to ensure patient care meets professional nursing standards and company policies and procedures. /li liCollaborating with the service delivery team to deliver client care plans. /li liPerforming delegated acts with the primary care team as needed. /li liPartnering with external service providers to coordinate and support integrated care. /li liDocumenting identified problems using a specified framework. /li liSelecting and implementing strategies or interventions from established care plans alongside clients, caregivers, and the health care delivery team. /li liUsing adult learning principles and established training materials to teach and enhance service provider skills. /li liMeeting with individual team members to create education and learning plans and contributing to annual PDP. /li liActing as a clinical resource for the service delivery team. /li liDemonstrating effective time management, planning and organization of daily activities for oneself and team members. /li liCollaborating with clinical and non‑clinical staff, allied health, community partners across the Peel shelter system, and other partners to advance the care model and clarify roles. /li liParticipating in staff planning and orientation of new staff, including EMR training. /li liManaging first‑line risk mitigation strategies for client or staff incidents. /li liSupporting the integration of WellFort CHS and other community services staff into a unified team. /li liBridging programming between the Safe Consumption Site, Bloom program, Homeless Health programming, and the comprehensive WellFort CHS services to support an integrated model of care. /li /ul h3How to Apply /h3 pApply by 08/31/2026 at /p /p #J-18808-Ljbffr
