OverviewFunctional Pathways is hiring a Physical Therapist for Daughters of Israel in West Orange, NJ. Offering a $5,000 Sign-on Bonus.ResponsibilitiesScreen all residents in the facility on a quarterly basis to identify declines in functional abilities and determine if Physical Therapy intervention is residents per MD order and establish appropriate Treatment Plans and Goals that meet the individual’s needs.Reassess patients and caseload at least every 30 days to determine the need for continued services or discontinuation of services; carry out the treatment plan as treatment implementation carried out by Licensed Physical Therapy Assistants; determine appropriateness and modify the treatment plan as needed.Provide thorough discharge planning to enable the patient to function at a maximal level of independence in the least restrictive environment.Educate staff, family, and caregivers as needs for program development and implement programs according to company policies and procedures.Identify equipment needs of the Physical Therapy Department and order equipment according to company and facility policy.Complete all required documentation according to guidelines in the Documentation Manual.Coordinate in-services for facility staff to ensure safe and effective carry over of Maintenance/Restorative Programs and overall care of the residents.Attend facility clinical required meetings (Medicare, Care Plans, etc.).Promote the profession within the facility, including assisting with coordinating special events (e.g., National Physical Therapy Month).Attend Continuing Education courses specific to the current position.Maintain professionalism and follow all applicable national/state student affiliates as directed.Assist employees in the absence of the Clinical Manager.QualificationsBachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.Certification and licensure requirements as established by state and national board associations; active state license required.Excellent organizational and time management skills.Excellent written and verbal communication skills.Working knowledge of Functional Pathways policies and procedures, and clinical programs with application to therapy disciplines.Ability to supervise Physical Therapy Assistants.Completion of all required screenings, certifications, and licensure (variations by location).Please note this is a job summary, not an all-inclusive listing of tasks and duties for this position.
