The Team Lead serves the Hospice team by utilizing advanced assessment, teaching and decision-making skills and professional and effective leadership skills, all while keeping the individual patient goals of care in the forefront. The Team Lead documents in the medical record in a thorough and timely manner according to agency requirements. Th Team Lead assists in facilitating team meetings. The Team Lead performs Administrative On-Call responsibilities, Holiday staffing and assists with emergency preparedness needs.Other Items of Note:

Required Education:Associates Degree in NursingPreferred Education:Advanced education or certification in area of specialty; Wound Care certifiedRequired Skills:Nursing skills as defined as generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with acute and chronic illnesses. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
Preferred Skills:
Home Care Home Base experience; Experience with smart phones and smart devices; Experience with Microsoft OfficeRequired Licensure/Registration/Certification:Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.Preferred Licensure/Registration/Certification:Wound Care Certified; Advanced education or certification in area of specialty.
Required Experience:
Two years’ experience in healthcare setting in respective disciplinePreferred Experience:Medical-Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care or Hospice experience.
