Shift Information:This position is PRN working as needed by the department. The Hospice weekend RN is a resource for hospice regulations, hospice criteria, hospice care and services and hospice philosophy for patients, caregivers, providers, Medical Director referral sources and community resources and provides education and consults to those in need. The Hospice weekend RN is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills during afterhours as designated. The Hospice weekend RN provides patient care based on provider’s orders, departmental policies and procedures, regulatory guidelines and best practice. The Hospice weekend RN provides care in collaboration with the provider, Medical Director, patient, caregivers, referral sources, community resources and Hospice IDG members to meet the goals of care with the consideration of the patient’s limitations and personal preferences in accordance with hospice philosophy. The Hospice weekend RN documents in the patient’s medical record in a thorough and timely manner according to agency requirements. The Hospice weekend RN attends and actively participates in team meetings. The Hospice weekend RN cares for the hospice patients by developing the plan of care in collaboration with the provider, Medical Director, patient, caregivers, referral sources, community resources and Hospice IDG members.Other Items of Note:

Required Education:Graduate from an accredited Diploma of Nursing Program or Associate’s Degree in Nursing.Preferred Education:Bachelor’s Degree in Nursing
Required Skills:
Nursing skills as defined by generally accepted standards of practice. Basic knowledge and skills to care for the clinical needs of the population served. Professional and efficient verbal and written communication skills. Good interpersonal skills. Ability to multi-task, basic organizational and time management skills. Basic computer skills. Knowledge of general office and administrative practices and procedures. Knowledge of how to utilize office equipment (copy machine, fax machine, etc.) Ability to be open, sensitive, flexible and ecumenical. Must have demonstrated experience in working with patients and caregivers dealing with life-threatening illness and end of life. Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.

Home Care Home Base experience; Experience with smart phones and smart devicesRequired Licensure/Registration/Certification:Current Texas state license in appropriate discipline; Current American Heart Association CPR; Current Texas state driver’s license; Current auto insurance. Must have reliable transportation.Preferred Licensure/Registration/Certification:Hospice and Palliative Care certified; Wound Care Certified; Advanced education or certification in area of specialty.Required Experience:Two years’ experience in healthcare setting in respective disciplinePreferred Experience:Medical-Surgical experience; Experience in Case Management; Supervisory position in healthcare setting; Home Care or Hospice experience
