LAMP provides a wide range of programs to improve health and well-being, connecting people to medical services and community programs. /ppThe WTOHT aims to improve healthcare experiences and outcomes in West Toronto by bringing together patients, caregivers, and providers to create a connected health and social care system. /ppbJob Summary /b /ppThe Project Manager will oversee the implementation of LDPHM, collaborating with partners, community organizations, and grassroots groups to promote preventative care, health promotion, and primary healthcare access.
